Program Change/Declaration

Before you begin the process to change your major, please note the following:

If your current program is non-degree you must change your status to degree seeking through the application for admission process.
If you are undeclared you must successfully remediate your restrictions. For more information please contact your advisor in the Academic Advising Resource Center at (859) 572-6900.
Reciprocity of Ohio excludes all associate programs, and the bachelor programs of Early Childhood Education, Environmental Science, Justice Studies, Nursing, and Pre-Nursing.

Download the Program Change/Declaration form (You must have Adobe Acrobat Reader in order to view and print the form.)
Print the document.
Complete the personal information section.
Complete the major, minor, or concentration sections.
Secure the appropriate approvals from all departments and submit the completed form to the Registrar's Service Center in AC 301.
Each department must make a photocopy if they need to retain the information on the forms for student advising files. The Registrar's Service Center will retain the original copy.
