Medical Records
To obtain a copy of your medical records, you must sign a medical release form provided in our office. Proper ID is required.
OR
Fax a release statement to our office (859.572.5615) with the following information:
- Your Name
- Social Security Number
- Name, address, and phone number of the person you want the information sent to
- A phone number where we can reach you to inform you that your information has been sent
- Your signature
