NKU Prospective Students:
To receive accommodations, students with
disabilities must register with Disabilities Services and provide current
documentation of a disability.
Students delaying registration and request for accommodation may delay their
access to accommodations. Professors will require that a student's need for
classroom and/or exam accommodations is verified through Disability Services.
To Register:
1. Complete your Admission to NKU
2. Contact Disability Services for an intake appointment 859/572-6373, disability@nku.edu or stop by the University Center room 320.

Complete and return the appropriate forms and return them to Disability

Documentation Verification Form
Note: Documentation of a disability should be current (within the last 3-5
years) and provided by an appropriate licensed professional. Learning
disabilities generally require a multi-factored psychological evaluation within
the last 5 years and students are encouraged to have these evaluations performed
using adult assessments. The Kentucky Association on Higher Education and
Disability (KY AHEAD) has published information approved assessment instruments.

4. Disability Services will contact you for an accommodation review and orientation after we receive your forms and documentation.
Planning for reasonable accommodations is an important step for a smooth transition to college. Reasonable accommodations are provided at no expense to the student and are based upon the individual student’s need and documentation. Reasonable accommodations will be discussed with the Director, Sue Roth (859) 572-5180, University Center, room 320.
5. To continue to receive accommodations, you must contact Disability Services with your new class schedule each semester.
